As far as the heart attack at 39, cocaine seizures are usually (99%) from rocking it up. It's immediate and if you have any kind of heart irregularity/murmur and it hits at the right time, you'll go into cardiac arrest.
When I was 24 (1986), we happened upon a 1/4 pound of the stuff. After two days, we couldn't snort any more and rocked it up. I passed my buddy the next hit, he went into a seizure. We got him on the ground, made sure he didn't swallow his tongue, and I gave him CPR (bruised his chest). We called the paramedics and by the time the arrived, we had him up and walking. Paramedics said we saved his life. next day, he was back smoking again.
About 2 months later, a friend was in an arcade, took a hit behind a machine and croaked. He was 23.
I still snort now and then but rocking it up and hitting the pipe is immediate and can be lethal. See Len Bias.
