So what about the ones that really are 100% passable?  How can you spot them?  

In any case my ex-girlfriend was usually only picked out by about 50% of Thai's.  Mostly I would say because of her height  5'7"  -  1.70m.

In the UK she was only spotted twice for sure and maybe a third time but I'm not sure.
As far as I know she was never spotted in any of the other countries we went to.
We must have met many many hundreds or even thousands of 'dumb' people.

Well the example I gave was tiny (around 5 foot tall ) had breast surgery and her adams apple shaved down , she actually wanted to go to a gay club one night and the doorstaff wouldn't let us in , when she told them she was a pre-op transsexual they laughed and said "nice try " .

Most westerners have never met a ladyboy in the flesh and wouldn't have a clue how the spot them .

I've mates who have been to Thailand a few times and aside of the more obvious ones have been totally fooled , I remember one guy getting totally excited over little Emma/Amy /whatever on walking street , and another mate almost cumming in his pants over a post op LB in Phuket .

A lot of us here are pretty good at spotting LBs ,yet every time I go I'm still fooled the odd time , a ladyboy I know pretty well introduced me to some friends outside of the bar scene , some GGs some LBs and all dressed in everyday clothing it was very hard to tell .

So I would say your 99.9999% is way off the mark even for those who have been to LOS many times , as for introducing a girl back home to someone who has never met a LB in their life , I would say the odds are firmly in the LBs favour .

IMO most Thais will spot a LB a mile away but will be too discreet to tell you. Except for the really obvious examples most Westerners will fail to spot the vast majority of LB's.

We can spot them because many are dressed like bargirls. Its the ones in the real world that you cant spot. Not the LB's that try too hard to be feminine. I have seen some that I swear are girls and I can spot them within minutes.

These girls are starting to take hormones as kids so it intercepts their puberty and makes them almost complete women. If you put them in a dress and have their makeup done with more natural tones, they look 100% from time to time.

Look at the face of Top from Guess Bar. Just focus on her face from the photo of my report. No way in hell could you tell she was a ladyboy if you put a dress on her.
